Services Provided
The Older Adult Health Hub provides a single-entry access point, offering coordinated assessment, care navigation, and linkage to clinical, community, and social support services through a multidisciplinary team.
The Hub provides integrated wraparound support, including health education, chronic disease self-management, allied health connections, carer support, and referrals to aged care, community programs, and wellbeing services.
Services include:
- Geriatric Assessment and Management
- Care Navigation
- Physical and Frailty Assessment
- Memory and Cognition Assessment
- Polypharmacy
- Social and Aged Care Assessments
- Advance Care Planning Support
- Aged Care Assessments
- Bone Health
- Older Adult Mental Health
Eligibility Requirements
Patients require a referral from their GP.
- Aged 65 years or over (50 years or over for Aboriginal people)
- Living within the South Metropolitan Health Service catchment area
- At risk of acute deterioration of frailty conditions, severe frailty, or carer breakdown
- Experiencing early to mild frailty, multiple risks, or recent functional decline
- Living with complex or chronic conditions, or geriatric syndromes, that would benefit from short-term multidisciplinary care and/or care coordination
