As a courtesy to our service providers and other clients Failure to attend 2 appointments will result in the client being removed from the service.
This decision has been made to encourage attendance and allow for other clients to access the service. We understand that in some instances appointments can be forgotten or circumstances my impact ability to attend. Therefore, if a client fails to attend 2 appointments this will result in the client being removed from the service.
Cockburn Healthy Lifestyle Service uses automated SMS reminders 72, 48 and 24 hours prior to appointments to assist clients in attending their appointment. Clients will be sent an email after their first DNA notifying them of this process.
Cancellation Policy
All CHLS health consult cancellations require a 24 hour notice prior to appointment. Clients are encouraged to give as much notice as possible to cancel their appointment. This will allow CHLS to fill the cancelled timeslot and provide care for another patient that may not otherwise be seen.
