Program Confirmation and Cancellation Protocol

To ensure all our clients experience our programs equally, clients who are referred to our programs at their health consultation will now be provided a program confirmation deadline in their program confirmation email that is sent upon referral. Clients will be notified at their health consult to check their email inbox folder and junk folder to the email address they provided the CHLS team when registering for the CHLS. If the email has not been received, it is then up to the client’s discretion to contact the CHLS team if a program confirmation email has still not been received.

Registered program clients will be sent a program reminder text 14 days prior to program commencement. Due to the high demand of programs, clients who fail to confirm or cancel their place on the program will have their place offered to clients on the program waitlist.

Clients who continue to fail to confirm their place for two programs, will be removed from the service unless alternate arrangements have been made with the CHLS team.

  • Upon referral to a program, referred clients receive an email with program registration details which will include a date to confirm their place for the program.
  • Clients are sent a reminder text 14 days prior to program commencement where the client can confirm their place on the program. Due to the high demand of programs
  • If clients have not confirmed or paid by this date, or alternate arrangements have not been made with the Cockburn Healthy Lifestyle Service team, their place will be offered to clients on the waitlist.
  • For paid programs, clients who have not paid by the payment deadline will be contacted by email/phone by CHLS Assistant Coordinator one week prior to commencement of the program.
  • Due to the high demand of programs, if the registered client has still not paid, their place on the program will be offered to participants on the waitlist unless alternate arrangements have been made with the CHLS team.
  • Clients who fail to confirm for two programs, despite the above reminder service and without valid reason will be removed from the service.
